What is an Incident?

An incident refers to any event that shouldn't occur, such as:

  • Harm or potential harm to you or others

  • Damage or loss of property

  • Situations where someone is injured or could have been injured

  • Serious issues like abuse, neglect, or significant injuries

Types of Incidents

Incidents can take many forms, including:

  • Experiencing or witnessing harm or abuse

  • Property damage or loss

  • Medical errors, such as incorrect medication administration

  • Sudden changes in your support that could impact your well-being

  • Illegal activities

How to Report an Incident

If you or someone else is involved in an incident, it's crucial to report it immediately. You can:

  • Speak directly with a Nineveh Care staff member

  • Send us an email

  • Call us

Our Response to Incidents

We take every incident report seriously and strive to respond promptly. For less serious incidents, our team will work closely with everyone involved to find a solution. For more severe or “reportable incidents,” we are required to notify authorities such as the Police and the NDIS Quality and Safeguarding Commission.

Information We Need

To support you effectively, we need specific details about the incident, including:

  • A description of what happened

  • When and where the incident took place

  • Who witnessed the incident

  • When it was reported

How We Support You

Your well-being is our priority. After an incident, we will:

  • Ensure your immediate safety

  • Support your health and recovery

  • Collaborate with you and your advocate to address the incident

  • Implement changes to prevent future incidents, such as updating policies or staff training
