The NDIS (National Disability Insurance Scheme) provides funding to eligible individuals with disabilities to enhance their quality of life. To be eligible for the NDIS, you must be aged between 9 and 65 years old. If you're under 9 years old, an early childhood partner can assist in determining if the NDIS is suitable for your child and provide support beforehand. Additionally, you must be an Australian citizen, permanent resident, or hold a Protected Special Category Visa, and reside in Australia. Eligibility also requires having a permanent and significant disability that impacts your daily life.

To apply for NDIS funding, you need to provide a completed access request form along with supporting information about your age, residency, and disability.

Here's how you can apply:

Call the NDIA: Contact the NDIA at 1800 800 110, and they will complete the form for you over the phone.

Fill Out the Form Yourself: Download and complete the access request form from the NDIS website.

Section 2 of the form must be filled out by your treating health professional unless you have existing reports or assessments that can serve as evidence.

The types of supports that the NDIS may fund for participants include:
