Understanding Your Service Agreement with Nineveh Care

What is a Service Agreement?

A service agreement is a plan between you and Nineveh Care. It details the support and services you’ll receive from us.

Our Responsibilities to Each Other

Responsibilities are tasks or things we agree to do. There are things you need to do, things we need to do, and some things we have to work on together.

Your Part

  • Let us know how you want your supports delivered.

  • If you need to cancel an appointment, please tell us 48 hours beforehand.

  • Inform us right away if there’s any change to your plan or if it ends.

Our Part

  • We’ll provide you with the supports that meet your needs, when you need them.

  • You’ll receive an invoice for your supports at least once every two weeks.

  • We follow all NDIS laws and regulations.

  • We’ll keep all the important paperwork.

  • If we need to end this agreement, we’ll give you 4 weeks notice.

  • We’re here to listen to your feedback and solve any problems quickly.

Working Together

  • Treating each other with kindness and respect is important.

  • We’ll make a plan for your supports together.

  • We’ll check on how your supports are going at least once a year.

  • Let’s keep communicating about your supports and funding.

Paying for Your Supports

There are a few ways your funding can be managed:

  • Plan Managed: If you have a Plan Management Provider, they will pay us.

  • Self Managed: You’ll manage your own funding and pay us directly. Don’t forget about travel costs if you’re managing your own transport funding.

We’ll send you invoices, and you’ll need to pay them within 7 days to continue receiving support.

Changing or Ending This Agreement

Sometimes, we might need to change or end this agreement. We promise to discuss any changes with you. If you or we decide to end the agreement, we need to give each other 4 weeks notice.

Cancelling Services and Giving Feedback

If you need to cancel, please provide 48 hours’ notice. Your feedback is very important, so feel free to share your thoughts or make a complaint if something’s not right.
